Functional Capacity Assessments

At Based., our Occupational Therapy assessments are built on best-practice clinical frameworks and tailored to meet NDIS requirements. We use formal, validated assessment tools to ensure accuracy, consistency, and clarity — all while keeping the process person-centred, respectful, and meaningful.

Assessments are available in person (Perth) or via telehealth (Australia-wide).

All Packages Include:

  • In-person assessment

  • Use of NDIS-recognised formal assessment tools

  • Review of reports, therapy input, and supporting documentation

  • Collaboration with participants, families, and support coordinators

  • Structured, clinically reasoned reports written in NDIS-aligned language

If a report is required urgently due to an upcoming plan review or time-sensitive request, we can accommodate faster turnaround times where possible — while maintaining the quality, accuracy, and depth of our work. Our Assessment Packages

Functional Capacity Assessment (FCA) – 12 hours

A comprehensive assessment across daily living, communication, cognition, mobility, and social participation. Commonly used for plan reviews, NDIS access requests, or changes in circumstances.

FCA + STA (Short Term Accommodation) – 16 hours

Includes a full FCA plus assessment and justification for STA funding to support respite, explore new routines, or build independence in a supported setting.

FCA + ILO (Individualised Living Options) – 25 hours

Combines the FCA with ILO exploration and mapping. Includes assessment of informal supports, housing goals, safety needs, and the development of a personalised ILO Support Model.

FCA + SIL (Supported Independent Living) – 25 hours

Focuses on functional skills, supervision requirements, and skill-building capacity. Includes a detailed report aligned with SIL funding criteria.

FCA + SIL/SDA (Specialist Disability Accommodation) – 30 hours

A high-level assessment addressing both SIL support needs and SDA eligibility. Includes detailed housing goals, environmental needs, functional impact, and alignment with the NDIS Home & Living guidelines.

Formal Assessment Tools We Use

Our assessments are tailored to each individual and may include one or more of the following evidence-based tools:

  • WHODAS 2.0 – Assesses disability and functioning across six domains of life

  • MoCA – Montreal Cognitive Assessment for attention, memory, and executive functioning

  • PEDI-CAT – Measures self-care, mobility, social function, and responsibility (for younger participants)

  • Vineland Adaptive Behaviour Scales – Evaluates daily living, communication, socialisation, and motor skills

  • Life Skills Profile (LSP-16) – Assesses functioning in mental health and psychosocial contexts

  • DASS-21 – Measures levels of depression, anxiety, and stress

  • COPM – Goal-setting and outcome measure based on client-identified priorities

  • Care and Needs Scale (CANS) – Assesses the level of personal support required in daily life (often used post-ABI or with high physical care needs)

  • Lower Extremity Functional Scale (LEFS) – Measures mobility and physical capacity in the lower body

  • Other standardised tools may be used based on the individual's presentation and goals
