Therapy Services

At Based., we offer a range of ongoing allied health services to support NDIS participants in building independence, wellbeing, and quality of life. Our team provides Occupational Therapy and Physiotherapy sessions focused on functional skill development, mobility, and capacity building. We also offer comprehensive support with Assistive Technology and equipment prescription. In addition, our Registered Nurses deliver respectful continence assessments to support health, hygiene, and dignity across all ages.

Occupational Therapy

Our Occupational Therapists support individuals to build independence, emotional wellbeing, and confidence in daily life. Services include:

  • Functional skill development (e.g. personal care, cooking, hygiene, routines)

  • Emotional regulation and sensory processing strategies

  • Psychosocial support, including routine-building, goal setting, and participation

  • Support for people with mental health challenges or psychosocial disability

  • Cognitive support, memory strategies, and dementia-informed care

  • Home safety assessments and environmental modifications

  • Assistive Technology (AT) and equipment assessment, prescription, and training

  • Collaborative support planning with families, carers, and multidisciplinary teams

Physiotherapy

Our Physiotherapists support NDIS participants to improve physical function, mobility, and independence through capacity-building therapy that aligns with their NDIS goals. We work with individuals across a range of physical disabilities, neurological conditions, chronic health issues, and age-related changes.

Services include:

  • Functional assessments to inform goal-based therapy and NDIS planning

  • Strength, balance, and endurance training to improve daily mobility and reduce falls risk

  • Gait retraining and movement support to enhance safety and independence at home and in the community

  • Pain management and reconditioning programs to address deconditioning and support sustained activity

  • Prescription and training in the use of assistive technology and mobility equipment (e.g. walkers, wheelchairs)

  • Rehabilitation following injury, surgery, or illness to restore and maintain function

  • Collaboration with support teams to embed therapy strategies into daily routines

Our Physiotherapy services are delivered in a way that is practical, person-centred, and outcomes-focused — helping participants maintain or improve functional independence in line with NDIS Reasonable and Necessary criteria.

Continence Nursing

Our Registered Nurses provide respectful, age-appropriate continence support for NDIS participants across the lifespan — from early childhood through to older adulthood. All assessments and reports are completed in line with NDIS guidelines to support funding applications, assistive technology requests, and plan reviews.

Services include:

  • Comprehensive bladder and bowel assessments tailored to individual routines, diagnoses, and developmental stages

  • Development of personalised continence care plans focused on dignity, independence, and quality of life

  • Detailed reports to support NDIS plan reviews, assistive technology (e.g. commodes, pads, urinals), and consumables funding

  • Education and training for families, carers, and support staff on continence management strategies

  • Risk reduction strategies to address skin integrity, infection risk, and continence-related distress

  • Collaborative planning with allied health professionals and medical teams where appropriate

Our continence services focus on functional outcomes, comfort, and long-term support — helping participants and their teams manage continence needs with confidence and dignity, both at home and in the community.
